I need to recertify but I don't know my FCSRH/MCSRH/CoSRH Diploma or LoC number - where can I find this?

We will need to be able to identify you easily when you submit your recertification forms.

You can give us:

• Your GMC or NMC number
• Your membership number (this begins with a 1 and you can find this at the top of the page when you log into 'MyCoSRH)
• Your qualification number (this could begin with a 'D' or 'A' or 'M' or 'F')

You can find your qualification number either on your initial certificate, recertification confirmation letter or by logging in here: Sign In | CoSRH to the CoSRH member's area of the website, or register here if you have not logged in before Register | CoSRH
